Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency currently seeking UAS-NY certified Registered Nurses to perform UAS Assessments in Manhattan, Queens, Brooklyn and the Bronx.

Responsibilities of the UAS RN:

• Assess eligibility of Medicaid members in need of managed long-term care using the UAS-NY web-based system.
• Conduct initial assessments and re-assessments in member’s homes or at skilled nursing facilities, according to state guidelines, policies and protocols.
• Consult with health care providers and review medical records and medications to support assessment outcomes.
• Provides education to members, caregivers regarding health issues, preventive care, medication management and plan benefits.
• Review and revise assessments as necessary based on company policy.
• Conducts quality checks on all completed documents before finalizing and sending.
• Abide by and promote Agency administrative policies.
• Attend meetings and training as required (webcam or in-person).
• Maintain current and active Health Commerce System (HCS) access.
• Maintain a continuing knowledge of competencies related to the nursing profession by participating in formal education programs, conferences, workshops, and professional organizations.

Qualifications

• New York State Registered Nursing (RN) license in good standing.
• UAS-NY Certification
• Proven knowledge of UAS-NY Assessments and proficiency in use of UAS-NY.
• One year nursing experience in Home/Community based or Long Term Care services delivery preferred.

Skills Required

• Solid assessment and documentation skills.
• Ability to effectively communicate verbally and in written formats at all levels.
• Ability to use computerized systems such as Microsoft Outlook, Microsoft Word, Excel.

Bilingual Spanish, Russian, Cantonese, Mandarin a plus.
