Social Worker - Full-time and PRN
For over 35 years, The Hospice of Baton Rouge has been meeting the end-of-life care needs of individuals throughout our 7 parish area and is the oldest continuously operating non-profit hospice provider in Baton Rouge.
The Hospice of Baton Rouge is seeking a PRN LMSW or LCSW. This position works weekdays and some weekend availability.
Qualifications:
· Current LMSW or LCSW License
· Hospice Experience Preferred
Responsibilities:
· Meets regularly with the Hospice team and assists with the developing and revising the patient/family care plan.
· Assess the patient/family psychosocial needs and coping skills including their response to terminal illness.
· Provides counseling services for the patient and family and other community individuals or groups who need psychosocial support in dealing with death and dying.
· Evaluates financial and other tangible needs and provides assistance with identifying, contacting, and receiving help from community resources as needed.
· Assists patients and their families with end-of-life decision making.
· Maintains accurate, up-to-date records of assessment and follow-up visits as well as any contacts made with patients and their families.
· Attend IDT Team and care plan update meeting weekly.
Job Types: Full-time, Part-time
Pay: $25.00 - $29.00 per hour
