Sapphire Rehabilitation of Goshen, a premier long term skilled nursing facility located in Orange County, New York is actively looking for a caring, dedicated Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

We are currently paying a $1,500 signing bonus for this position (to be paid after successful completion of 90 day probationary period).

$1,200 Referral Bonus paid out quarterly over the year

PT Duties Include:

• Supervise the activities of physical therapy assistants.
• Evaluate Residents conditions & develop appropriate physical rehabilitation plans.
• Help Residents improve their mobility.
• Relieve Resident’s pain using exercises, hands-on therapy & stretching.
• Teach Residents how to use physical therapy equipment.
• Help Residents through their tasks and monitor their performance.
• Maintain Resident confidence & protects the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort.
• Submit all billing logs and necessary information on a timely basis.

PT Requirements Include:

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Nursing Home Experience preferred
