The Navigator is responsible for identifying the appropriate placement and care for individuals experiencing a change in health condition or residential status that are part of or are seeking services within the Milwaukee Catholic Home Continuum and through home transitions. This individual, working with appropriate team members, supports the development of a discharge/ transitional plan of care when necessary while directly supporting the individual and the family through transitions of care. Transitions may include, but are not limited to:
· A resident in the independent setting that is showing signs of a medical/social/psychological change of condition that is not part of the RCAC.
· A resident in the independent setting that went to the hospital and is seeking services in MCH’s Health Care Center.
· A resident in the independent setting that went to the hospital and is seeking supportive home care services in the independent residence setting.
· A resident in Transitions Care seeking services in long term care or the CBRF setting.
· “Friends” of the MCH who are considering moving into any of the settings at MCH.
· Professional priority contacts that are identified.
ESSENTIAL FUNCTIONS
This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.
1. Presents the baseline care plan, to residents being actively served by the Navigator, within 48 hours of admission to post-acute care.
2. Maintains a list of residents in transition and maintains a record of their current health status and needs. Facilitates care conferences regarding residents who have been or are actively being served by the Navigator.
3. Communicates and coordinates effectively with the interdisciplinary teams to allow smooth transitions of the resident between areas of care or to their home. Presents the final discharge plan to resident and family.
4. Communicates effectively with the resident and family members throughout their involvement with MCH and post discharge.
5. Assist with assessing and providing the needed resources and support to residents so they can thrive and acts as an Advocate when necessary to address concerns or other requests as indicated by resident and family.
6. Accountable for Navigator paperwork and appropriate documentation.
7. Ensure all areas of care are providing programming to meet the needs of the residents within the continuum of care.
8. Provide continued quality assurance and assessment of programs offered (QAPI).
9. Stays updated with industry regulation by developing and maintaining networking groups and attending conferences.
10. Other duties as assigned.
Education and/Or Experience
Three to five years of experience in long-term care setting preferred. Bachelor’s or Masters degree within the Health Services Field, preferably Social Work, Occupational Therapy or Nursing. Extensive knowledge of Physical Rehabilitation, adaptation, advocacy, and community resources.
Knowledge
Knowledge of available senior resources
Knowledge of community resources and services available for seniors
Knowledge of support services available for family members
Knowledge of facility and state mandatory reporting practices.
Knowledge of social work standards of practice, ethics, and policies.
Knowledge of the problems and issues facing the geriatric population and their caregivers.
Knowledge of basic counseling skills and techniques.
Knowledge of PPS,MDS, care plans, and medical terminology.
Knowledge of MS Office, including Word and Outlook.
Knowledge of structure and content of the English language including meaning and spelling of words, rules of composition, and grammar.
Job Type: Full-time
Pay: $48,000.00 - $55,000.00 per year
