Job Summary:
The Nurse, assumes primary leadership accountability and responsibility for milieu management as well as directing client care activities of other nursing staff. The nurse maintains standards of professional nursing practice in accordance with facility policy and procedures, other external governing and credentialing bodies, performance improvement standards, and psychiatric nursing standards. The nurse assesses the client utilizing the nursing process to formulate an individualized plan of care for each client, implements interventions, and evaluates outcomes consistent with policy and appropriate to the client’s condition. Establishes short and long term goals which promote, maintain, and restore physical and mental health, leading to abstinence and recovery. The nurse interprets age/disability specific information in response to client population served. The nurse provides for the safe delivery of medication in accordance with the policies and procedures, physicians orders and/or current practice. The nurse is responsible for thorough, accurate reporting and documentation of the client’s symptoms, responses and progress. The nurse assumes responsibility for professional development, continuing education and maintaining licensure.
Job Duties:
UTILIZES THE NURSING PROCESS in assessing, planning, and formulating nursing interventions directed towards development of an individualized plan of care and treatment modalities
PERFORMANCE STANDARDS DEMONSTRATES proficiency in psychiatric clinical skills
- Conducts ongoing assessment of clients from admission to discharge
- Identifies expected outcomes individualized to the client to include discharge planning
- Prescribes interventions to attain expected outcomes
- Evaluates the effectiveness of interventions in relation to expected outcomes
- Provides individual client teaching on all identified learning needs
- Facilitates in Family/Significant other education as required
- Performs vital signs/measurements on clients as ordered and documents same in medical record

IMPLEMENTS NURSING INTERVENTIONS identified in the plan of care that is individualized to the client needs. Applies appropriate interventions to progress the client towards wellness and addresses maintenance of wellness.
