The mission of Advantage Care Health Centers is to provide the highest quality comprehensive primary, dental, mental, and behavioral health care services.

We offer these services to all community members committed to those who might otherwise be excluded from the health care system while remaining cost-effective and efficient. The health center specializes in providing services to children and adults with intellectual and developmental disabilities.

In this role, you will:

• Administer injections, vaccination and medication as ordered by MD/NP/PA.
• Monitor inventory and order medical supplies and materials; monitor inventory (vendors, labs, pharmacy).
• Maintain log books for quality control.
• Provide preventive health education.
• Complete prior authorizations for medication, DME and services as requested.
• Perform medication reconciliation.
• Manage medical phone line.
• Preload intake packets for new patients.
• Supervise medical assistant staff and complete annual vital sign assessments and trainings as indicated.
• Provide educational support to patients and direct care professionals on behalf of patient.
• Assist new hires with EMR training, skills training, infection control training.
• Reconcile labs/referral orders electronically.
• Utilize Healthix to receive patient hospitalization, ER and rehab tests, consults, procedures and discharge summaries.
• Utilize NYSSIS for immunization tracking.
• Serve as a member of the Infection Prevention, Performance Improvement, and other committees deemed necessary.

We require:

• Valid LPN license from an accredited vocational training institution.
• 24 Continued Medical Education (CME) credits per credentialing cycle (2 years).
• Basic Life Support Certification; AED Certification.
• Strong computer, written and verbal communication and organizational skills.
• Prior experience coordinating medical record activities (EMR) of a busy medical office or health care center, preferred.
• Flexibility to work at both sites.
