JOB TITLE: Designated Coordinator

PROGRAM: Hearth Residential IHS Program

STATUS: Part-Time less than 20 hours per week

EDUCATION/ EXPERIENCE preferred: Bachelor’s degree (B.A., B.S.) from a regionally accredited college in a related area (i.e. Social Work, Psychology, RN). Prior supervisory or management experience important. 245D experience preferred.

Position requires a minimum of 2-4 years direct care experience with developmentally disabled population. Experience preparing and analyzing reports, supervision of staff and the ability to make decisions, as well as participate on the Leadership Team. Solid communication skills, staff management and public relations, organization skills, knowledge of computers and records management are all required for this position. Employment is contingent on providing agency evidence of related degree, clear background check and motor vehicle reports.

JOB SCOPE:

· At all times, ensure and advocate for the proper care and safety of all consumers.

· Directly perform hands-on work and attend training on an on-going basis to respond to consumer needs and otherwise role model appropriate behaviors in the agency.

· Provide positive, accountable leadership within the organization. Recognize and motivate direct reports; coach and train direct reports for operational excellence.

· Shall ensure that agency Safety policies are followed

· Perform duties within 20 hours per week.

CORE RESPONSIBILITIES:

· Develop and implement plans determined by the 245 D rules and regulations

· Act as the Designated Coordinator for all consumers assigned

· Be transparent, accountable and complementary to other Leadership Team members for all operations. Daily communication leadership team required

· Participate as interdisciplinary team member for all consumer reviews

· Follow all company Standard Operating Procedures.

· Provide direct care to consumers as needed or when requested

· Maintain expertise in behavior management/therapeutic intervention/CPI techniques.

· Assist staff trainers to develop and facilitate consumer-specific training.

· Present a positive role model to consumers with regard to communication, dress and behavior.

· Treat all consumers in an age appropriate manner, respecting their dignity and individual worth.

· Treat all family members and residential staff members with respect, in a compassionate manner at all times.

· Assume other duties as assigned by the Executive Director

PROFESSIONAL CONDUCT:

· Supports and participates in common teamwork; cooperates and works together with co-workers and

Leadership Team.

· Uses tactful, appropriate and professional communication.

· Promotes positive relationships among employees and with consumers.

· Maintains confidentiality relating to all company related information and matters.

· Shall be capable of relating to individuals at various levels in the company facility and out in the community as unique situations present themselves, the employee shall be sensitive to company and consumer needs, employee goodwill and public image.

TRAINING REQUIREMENTS:

· Medication Administration for unlicensed personnel

· American Heart Association Basic Rescue Certification

· Red Cross Basic First Aid certified
